The Case Management Program assists clients with navigating social and medical systems. The goal of the Non-Medical Case Management (Benefits Coordination) is to assist people living with HIV to address non-medical barriers that make it difficult to maintain treatment adherence. They also facilitate access to benefit programs and other available community resources. The goal of the Medical Case Manager is to increase positive medical outcomes, including viral load suppression, for people living with HIV. Client’s work with the Medical Case Manager in navigating medical and behavioral health systems; creating and maintaining a collaborative treatment plan. The Case Management team works closely with our medical team to support individuals and their partners in the agreement that Undetectable means Untransmittable (U=U).
