Early Intervention Services is a “one-stop-shop” approach to ensuring persons living with HIV and those behaviorally at risk for HIV are connected to care. It includes Outreach and Health Education, Targeted HIV testing, Linkage to Care Navigation, and referrals for medical and social support. Under this program, Heart to Hand’s Linkage to Care Navigators and volunteers provide HIV testing in the community on our mobile unit and onsite in our main office, facilitate health education sessions and work one-on-one with individuals to ensure they are linked to quality care including access to Pre-Exposure Prophylaxis (PrEP), a pill that can greatly reduce the chance of acquiring HIV.
